The Opportunity Alliance, in collaboration with Freeport Community Services, will be hosting two “blitz” days where they are accepting walk-in appointments for residents living Freeport and the surrounding area.
HEAP intake workers will be taking walk-in appointments at Freeport Community Center on the following days between 9am-4pm:

The following documents may be required:
Government-issued photo ID for primary applicant
Date of birth and documentation of Social Security number for all household members
Fuel vendor name and account number, name of person on the account
Proof of income for all household members for the 1 month or preceding 30 days.
Wages: all pay stubs with check date and gross pay within the income verification period
Social Security: award letter from Social Security. Call 1-800-772-1213 to get a copy of your award letter
Pension/Retirement: a check stub showing gross amount or an award letter
Self-Employment: your most recent signed federal tax return
Current utility bills: electricity (CMP), water (if applicable)
For renters: Please know your landlord’s name, address, and telephone number.
For subsidized housing: housing certification letter that is in effect on the date of application (HUD form 50059-50059 or RDD3560-8)
